What does low testosterone mean?
Low testosterone is not simply a result below a laboratory range. A diagnosis of male hypogonadism needs compatible symptoms or signs, low testosterone confirmed on a second morning test, and an assessment of the cause. Only then does it make sense to decide whether treatment is appropriate.
Usual testing window
The European Association of Urology recommends measuring total testosterone in the morning and, generally, fasting.
Confirm before treating
If the first result is low, it is normally repeated on another day before starting treatment.
A threshold, not a diagnosis
The European guideline uses this as a practical threshold in late-onset hypogonadism. Interpretation still depends on symptoms, the laboratory and other clinical information.

What are the symptoms of low testosterone in men?
Symptoms can affect sexual function, the body, mood and energy. Many are non-specific and can also occur with poor sleep, depression, obesity, other illnesses, stress or medicines.
More suggestive
- Persistent reduction in sexual desire
- Fewer spontaneous or morning erections
- Erectile dysfunction in the appropriate clinical context
Can also occur
- Fatigue or reduced vitality
- Mood or concentration changes
- Reduced muscle mass or strength
- Increased body fat
Low-testosterone symptom checker
If you are looking for a "low testosterone test", one point matters: no online questionnaire can confirm hypogonadism. This checker is designed to organise symptoms and context before speaking to a doctor.
How is low testosterone diagnosed?
Symptoms and history
The doctor reviews sexual symptoms, energy, sleep, medicines, weight, fertility and relevant testicular or pituitary history.
Morning blood test
Total testosterone is usually measured in the morning, generally between 07:00 and 10:00 and preferably fasting.
Repeat a low result
A low result is normally confirmed on another day before treatment is considered.
Find the cause
LH, FSH, SHBG, free testosterone, prolactin and other tests may be needed depending on the pattern.

0652 FAQs
Practical answers about testing, prescriptions and treatment in Spain.
What is TRT and when might it be considered?
Testosterone replacement therapy is used to treat confirmed hypogonadism: compatible symptoms plus repeatedly low testosterone after appropriate assessment. The goal is physiological replacement, not "optimisation" of a normal result.
How is a decision about TRT made?
The usual pathway is clinical: consultation, confirmatory blood tests, investigation of the cause, a safety and fertility review, then a shared treatment decision if TRT is appropriate. This can involve face-to-face care or, in some services, telemedicine, but being online does not remove the need for a genuine diagnosis.
Consultation
Symptoms, medicines, medical history, sleep, fertility and factors that can explain a low result are reviewed.
Confirmatory testing
A low morning testosterone is normally repeated; LH, FSH, SHBG, free testosterone, prolactin and a blood count may be added depending on the case.
Prescription and follow-up
If treatment is indicated, the doctor can prescribe an authorised formulation and arrange monitoring of response, testosterone, haematocrit and other relevant parameters.
Why can testosterone fall?
Low testosterone may reflect a testicular problem, reduced signalling from the hypothalamus or pituitary, or functional suppression related to factors such as obesity, illness, medicines, poor sleep or previous androgen use.
Testosterone and fertility: a conversation to have first
External testosterone suppresses LH and FSH and can markedly reduce sperm production, sometimes to azoospermia. If current or future fatherhood matters to you, say so before starting TRT.
Does testosterone fall with age? Is "andropause" real?
Testosterone can fall gradually with age, but there is no universal male equivalent of menopause. Age alone is not a reason to treat. Symptoms and repeatedly low testosterone are still required.
Common questions about low testosterone and TRT
Can I get TRT online in Spain?
Telemedicine can be part of assessment and follow-up, but it does not remove the need for appropriate blood tests, diagnosis, safety checks and clinical judgement.
Does TRT affect fertility?
Yes. External testosterone can substantially suppress sperm production. Fertility should be discussed before treatment.
Is 12 nmol/L automatically low?
No. It is a practical threshold used within a diagnostic process, not a stand-alone diagnosis.
Do I need two tests?
A low result is normally confirmed on another morning before treatment is started.
Terms worth separating
- Low testosterone
- A laboratory result below the expected range; not automatically a diagnosis.
- Hypogonadism
- A clinical diagnosis combining compatible symptoms or signs with confirmed biochemical deficiency.
- TRT
- Testosterone replacement therapy used for appropriately diagnosed deficiency.
- SHBG
- Sex hormone-binding globulin, a protein that affects interpretation of total testosterone.
- LH / FSH
- Pituitary hormones used to help identify where the problem is coming from.
